How this calculator works
Sepsis requires infection and an acute SOFA rise of at least two. The shock check adds vasopressor need, lactate above 2 mmol/L and adequate resuscitation. qSOFA is shown separately. This arithmetic check cannot establish a diagnosis. The result is an estimate for information only and is not medical advice.
Formula
SOFA change = current - baseline. Sepsis criteria: infection AND change >= 2. Shock criteria: sepsis AND vasopressor requirement AND lactate > 2 mmol/L despite adequate volume resuscitation.
Worked example
Infection with baseline SOFA 1 and current SOFA 3 meets the sepsis criteria. Lactate 3 with the required vasopressors after adequate resuscitation also meets shock criteria. Lactate exactly 2 does not meet the lactate condition.

Frequently asked questions
Can baseline SOFA always be zero?
Only assume zero when the patient is not known to have pre-existing organ dysfunction, as specified by Sepsis-3.
Does high qSOFA establish sepsis?
No. It is a risk flag and does not enter this sepsis criteria check.
Can a negative result exclude urgent illness?
No. The clinical assessment takes priority and the entries may change over time.
