How this calculator works

Check for interfering factors, observe, stimulate when appropriate, then rate each response. A total is shown only when all three components are testable. Retain the components when communicating the assessment. The result is an estimate for information only and is not medical advice.

Formula

GCS = E + V + M. Eye response ranges from 1 to 4, verbal from 1 to 5, and motor from 1 to 6. NT is a notation, not a numeric score.

Worked example

Spontaneous eyes, confused conversation and obeying commands give E4 V4 M6, a total of 14. E2 V2 M4 gives 8.

Frequently asked questions

How is an intubated patient recorded?

If intubation prevents verbal assessment, choose Not testable for verbal response. This page preserves VNT and withholds a numeric total.

Is a response absent or untestable?

Absent means the response was assessed and not observed. Untestable means an interfering factor prevented assessment. They are different findings.

Can the total replace the examination?

No. Different response patterns can have the same total. Sedation, language and injuries also affect assessment.
