How this calculator works
GCS contributes up to two points. Age, initial haematoma volume, ventricular extension and infratentorial origin contribute up to one each. This describes initial severity; it must not be used alone to limit treatment or predict an individual outcome. The result is an estimate for information only and is not medical advice.
Formula
GCS 3-4: 2 points; GCS 5-12: 1; GCS 13-15: 0. Add 1 each for age at least 80 years, ICH volume at least 30 mL, intraventricular haemorrhage and infratentorial origin.
Worked example
Age 80, GCS 10, volume 30 mL, ventricular blood and supratentorial origin give 1 + 1 + 1 + 1 + 0 = 4. Age 60, GCS 15, volume 20 mL and neither imaging feature give 0.

Frequently asked questions
Does 30 mL count?
Yes. The volume threshold is inclusive, as is the age threshold of 80 years.
Which GCS should I use?
Use the presentation GCS, or the assessment after resuscitation as specified in the original instrument. Record factors that interfere with assessment.
Where is the mortality percentage?
This page reports the score. Historical group outcomes are not a reliable personalised prognosis, particularly when care practices differ.
