How this calculator works
The original International Prognostic Index assigns one point each for age above 60, stage III or IV, LDH above the laboratory upper limit, ECOG performance status at least two, and more than one extranodal site. Its original aggressive-lymphoma cohort predates modern DLBCL treatment. The displayed survival percentages describe that 1993 cohort, not an individual forecast or current expected survival. This page is the original IPI, not revised IPI or NCCN-IPI. LDH is compared with the reporting laboratory limit rather than a fixed universal concentration. The result is an estimate for information only and is not medical advice.
Formula
IPI = sum of five binary risk factors. Groups: 0 to 1 low; 2 low-intermediate; 3 high-intermediate; 4 to 5 high. Original five-year overall survival: 73%, 51%, 43%, 26%.
Worked example
Age 60, stage II, normal LDH, ECOG 1 and one extranodal site gives zero points and the low group. Age 61, stage IV, raised LDH, ECOG 2 and two extranodal sites gives five points and the high group.

Frequently asked questions
Does age exactly 60 score a point?
No. The original age criterion is above 60 years.
Is 280 U/L a universal LDH limit?
No. Use the upper limit from the laboratory that performed the test.
Is the historical survival my chance today?
No. Treatments and populations have changed. Discuss current prognosis with the treating lymphoma team.
