How this calculator works

DASI is a weighted index, not a simple count of Yes responses. Enter responses already collected with the authorised 12-item questionnaire; the short item cues here do not replace that instrument. The Hlatky equation estimates peak oxygen uptake from the total and then expresses it in METs. Reverse modes only rearrange that fitted relation for a result already estimated by DASI. They must not treat a measured exercise-test value as an actual completed questionnaire score. No perioperative decision or individual fitness diagnosis is made from this page. The result is an estimate for information only and is not medical advice.

Formula

DASI = sum of the weights for affirmative items: 2.75, 1.75, 2.75, 5.5, 8, 2.7, 3.5, 8, 4.5, 5.25, 6 and 7.5. Estimated peak VO₂ = 0.43 × DASI + 9.6 mL/kg/min. Estimated METs = VO₂/3.5.

Worked example

All 12 affirmative responses give 58.2, estimated VO₂ 34.626 mL/kg/min and 9.893 METs. A recorded DASI of 20 gives VO₂ 18.2 and 5.2 METs.

Frequently asked questions

Are all activities equally weighted?

No. Each activity has its own published coefficient. The maximum total is 58.2.

Why is the questionnaire not printed in full?

The original instrument has its own wording and administration requirements. Use the authorised questionnaire to collect responses and this form to check the scoring arithmetic.

Is estimated VO₂ a measured exercise-test result?

No. It is a regression estimate. Direct cardiopulmonary exercise testing is a different assessment.
