How this calculator works
This scoring aid uses CMS C0200, C0300A-C and C0400A-C. Conduct the interview using the CMS script, repetition and cueing procedure before entering scores. Repetition is scored on the first attempt. Month orientation allows five days around a month boundary. An incomplete interview is coded 99, not 0; follow CMS instructions for inability to participate, nonsensical answers and refusal of four or more items. The interpretation uses the CMS clinical BIMS bands, not the differently labelled PDPM payment groups. This is a cognitive screen, not a dementia diagnosis. The result is an estimate for information only and is not medical advice.
Formula
BIMS = repetition (0-3) + year (0-3) + month (0-2) + weekday (0-1) + three recall scores (0-2 each), maximum 15.
Worked example
Repetition 3, year 3, month 2, weekday 1 and recall 2, 1, 0 gives 12/15.

Frequently asked questions
Can I enter calendar answers instead?
Select the CMS-coded response band after considering the interview date; do not subtract month numbers mechanically.
What does 99 mean?
The interview was incomplete. It is a coding value and is never added to the 0 to 15 scale.
Does this diagnose dementia?
No. Diagnosis requires a clinical assessment and consideration of delirium, hearing, language and other factors.
