How this calculator works

The original final logistic model uses five predictors and the coefficients in Table 2. Its endpoint is myocardial infarction or cardiac arrest through 30 days, not all complications or overall survival. NSQIP excluded trauma, transplant and patients under 16. Procedure classification and ASA class require clinical judgement. Changing the creatinine unit reinterprets the entered number. The result is an estimate for information only and is not medical advice.

Formula

L = -5.25 + 0.02 x age + functional-status coefficient + ASA coefficient + creatinine coefficient + procedure coefficient. Risk = 1/(1 + exp(-L)). Creatinine above 1.5 mg/dL adds 0.61; missing adds -0.10; normal adds zero.

Worked example

A 60-year-old independent patient in ASA IV having aortic surgery with normal creatinine has L = -5.25 + 1.20 - 0.95 + 1.60 = -3.40, giving about 3.23% risk.

Frequently asked questions

Is this the newer universal surgical-risk calculator?

No. This implements the specific Gupta 2011 MICA model, using its original procedure groups and endpoint.

What does missing creatinine mean?

The original model assigns a separate -0.10 coefficient to a missing result. It is not assumed normal.

Can this decide whether surgery is safe?

No. A probability estimate cannot replace clinical assessment, informed discussion or current perioperative guidance.
