How this calculator works

This implements the original 2008 binary screening rule for adults, with one point per criterion. Body size is calculated from the entered measurements. The original neck threshold is greater than 40 cm for either sex, age is greater than 50, and BMI is greater than 35. Some later forms use different neck criteria and risk groupings; do not mix versions. A positive screen calls for assessment, not a diagnosis. The result is an estimate for information only and is not medical advice.

Formula

Score = four yes-response points + I(BMI > 35 kg/m²) + I(age > 50 years) + I(neck > 40 cm) + I(male). BMI = weight in kg / height in metres². The original positive threshold is 3 of 8.

Worked example

A 45-year-old woman with BMI 25, neck 39 cm and no symptom points scores 0. A 60-year-old man with BMI 36, neck 41 cm and all four symptom points scores 8.

Frequently asked questions

Why is the neck rule the same for both sexes?

That is the original eight-factor publication used here. Later forms may use sex-specific collar sizes.

Does 50 years add a point?

No. The original age criterion is over 50; exactly 50 does not add a point. BMI exactly 35 and neck exactly 40 cm likewise do not meet the strict thresholds.

Can this diagnose sleep apnea?

No. Sleep testing and a clinical assessment are needed. Do not use the score to decide whether it is safe to drive when sleepy.
