How this calculator works
Complete an authorised ESS-CHAD questionnaire first, then transfer each recorded item score in its original order. All eight responses are required. The questionnaire is copyrighted and needs a licence for use; this page supplies arithmetic only. Adult severity bands are not assigned to children here. A clinician should consider age, sleep schedule and the reason for sleepiness. The result is an estimate for information only and is not medical advice.
Formula
ESS-CHAD total = item 1 + item 2 + item 3 + item 4 + item 5 + item 6 + item 7 + item 8. Each recorded item is 0, 1, 2 or 3; the total ranges from 0 to 24.
Worked example
Recorded scores 1, 0, 2, 1, 3, 0, 1 and 0 sum to 8. Eight scores of 2 sum to 16.

Frequently asked questions
Where are the questions?
Use the authorised form linked in the sources. Rewording the items would create an unvalidated questionnaire.
Can I leave one item unanswered?
No. Obtain all eight recorded responses before using the total; missing items cannot simply be treated as zero.
Does the total diagnose a sleep disorder?
No. It records reported sleep propensity across the form activities. Persistent sleepiness warrants professional assessment.
