How this calculator works

Choose a version before entering results. Adult MELD 3.0 uses sex, bilirubin, INR, creatinine, sodium and albumin. Values below 1 for bilirubin, INR and creatinine are set to 1; creatinine is capped at 3, sodium at 125 to 137 and albumin at 1.5 to 3.5. Qualifying dialysis sets creatinine to 3. Historical versions cap creatinine at 4. The displayed score is bounded 6 to 40 and rounded as required by the model. This page is restricted to adults registered at age 18 or older; it omits adolescent-specific allocation rules and personalised mortality percentages. The result is an estimate for information only and is not medical advice.

Formula

MELD 3.0 = 1.33(if female) + 4.56 ln(B) + 0.82(137-Na) - 0.24(137-Na)ln(B) + 9.09 ln(INR) + 11.14 ln(Cr) + 1.85(3.5-albumin) - 1.83(3.5-albumin)ln(Cr) + 6. Historical MELD = 10[0.957 ln(Cr) + 0.378 ln(B) + 1.120 ln(INR) + 0.643]. Historical MELD-Na adds 1.32(137-Na) - 0.033 x MELD x (137-Na) when rounded MELD is above 11.

Worked example

With bilirubin, INR and creatinine all 1, sodium 137 and albumin 3.5, adult male MELD 3.0 is 6 and adult female MELD 3.0 is 7.33 before rounding to 7.

Frequently asked questions

Is this an official allocation score?

No. Allocation policy includes eligibility, exceptions and recertification beyond the arithmetic.

Why does dialysis change creatinine?

The model substitutes its version-specific cap when the specified recent dialysis condition applies.

Why omit a mortality percentage?

A fixed risk table from an older cohort is not a personalised probability for current MELD 3.0.
