Fagerstrom test for nicotine dependence calculator

Sum the six original FTND item codes from Heatherton et al. (1991). The 0 to 10 score describes cigarette dependence, not a treatment dose.

How this calculator works

Use Table 3 of the original 1991 paper to answer and code all six items. Time to first cigarette scores 3 for within 5 minutes, 2 for 6 to 30, 1 for 31 to 60 and 0 after 60. Cigarette consumption scores 0 for 10 or fewer per day, 1 for 11 to 20, 2 for 21 to 30 and 3 for 31 or more. Each of the other four items is binary, 0 or 1, according to the original form. This is the six-item FTND, not the older eight-item Fagerstrom Tolerance Questionnaire. The original sum is reported without later severity bands or medication recommendations. The result is an estimate for information only and is not medical advice.

Formula

FTND = Q1 + Q2 + Q3 + Q4 + Q5 + Q6. Item maxima are 3, 1, 1, 3, 1 and 1; the total ranges from 0 to 10.

Worked example

Item codes 2, 1, 1, 1, 0, 0 total 5. Codes 3, 1, 1, 3, 1, 1 total 10.

Frequently asked questions

Is this intended for vaping?

No. The original questions concern cigarettes. Do not convert vape puffs, nicotine pouch use or other tobacco products into cigarette responses.

Why are severity categories absent?

The original paper proposes the scale and its item scoring. Common later bandings are not part of that original instrument and are not imposed here.

Does the score select a nicotine replacement dose?

No. Discuss cessation options with a qualified clinician or pharmacist. The score alone does not determine treatment.
